Add setting to remove assistant modifiers

Severals members have requested the ability to remove the defaulted assistant modifiers for different scenarios. Examples of these scenarios include: medicare/tricare, OT charges, hospital-based clinics that integrate with billing softwares which apply modifiers after the charges have been sent from WebPT, etc.
