I think that it would be great to have a "quick text" provider update capacity. I suppose it could be in a separate drop-down, or in the "case note", but a way of quickly updating the provider.

Dear Dr. X;

Your patient has attended X appointments. Skilled PT has included...

They have improved XXX. They still have difficulty with XXX.

Thank you ...
