Numbers added to outcome measure questions and answers

I would like to see all of the questions and answers on the outcome measures be numbered, for ease of input. The printed paper outcome measure questionnaires (i.e. the Oswestry, the LEFS, etc...) have each question and the answers to each question numbered. Our patients fill out the questionnaires by circling the number corresponding to their answer. We, the PTs and PTAs, are responsible for inputting each individual answer into WebPT for the questionnaires, and it is very easy to get "off" by a question when inputting, especially on some of the longer forms (like the LEFS). If the questions and answers were numbered like they are on the printed forms, input would be much easier. In my head, saying to myself "2, 2, 3, 1, 2" and then clicking those answers is easier than saying in my head "moderate, moderate, quite a bit, little, moderate" and then clicking those answers. So, a faster, easier, and more accurate means of input for functional outcome measure answers is what I'm looking for here.

A secondary request for the outcome measures is that there could also be a "select all" option, so that if a patient selected mostly "moderate" (or 3) on the QuickDASH, I could click "select all - 3", and than all I would have to do is go back and change a couple of answers that were different. Again, the LEFS input gets lengthy, and for patients who answer mostly 4 (no difficulty), but the default answer is 0 (extreme difficulty), I wish I could just select 4 for all answers, then go back and change the few that are different. And if each question were numbered, it would be easier to go back and change those few.

    This is something that we enhanced in our upgrade to documentation that we are working to roll out to all of our members. As you can see in the attached image you can see the number and you also have the ability to type the number select enter and tab to the next field to enter the score.


    An even better option is to have the patient complete the form digitally and then import the answers and score with a single click inside the documentation. Here is an article with more information on this option.
