Outcome measures - remove the mandatory follow up satisfaction questions required to calculate % disability

the most important part of these are tracking score / outcome. If a medicare patient doesn't complete those questions because they forgot or were given the initial evaluation form to complete (on paper which is how our clinic does this), I am unable to lock the note. I think satisfaction measures are great but the absence of an answer in this section should not prevent a provider from being able to finalize a note
