Allow transition from Discharge summary to Progress note

Allow transition from Discharge summary to Progress note without having to rewrite

Many times we don't know if the patient will be continuing or discharging prior to doing the ReEvaluation. But on WebPT you have to pick one or the other to open the note and if you choose the wrong one you'll have to delete and rewrite the whole thing. Other EMRs allow this
