Charts notes aren't really secure. You can enter a chart note and date it a week earlier or you can go into another users note and edit it.
Chart notes should be time stamped and not be allowed to be edited especially by another user.
Another system I used would have a section for their version of chart notes and it would be time stamped and permanent. The original person could only delete a their own note but if you needed to edit it you basically had to enter a new time stamped note.

In the meantime, if you are looking for more locked-down documentation you might consider using the case notes feature as they cannot be edited the way that chart notes are. Chart notes are typically used for leaving notes/reminders to other clinic staff, whereas the case notes is part of the clinical record and are more secure.
