Be able to open a new client from an unassociated clinic

Be able to open a new client from an unassociated clinic with a placeholder IE instead of having to re-type the other clinics eval.

Too much paperwork to recopy a paper eval from an unassociated clinic into the initial eval to open a new client case when the insurance does not pay for a second eval. But the program does not allow us to open a new client case without an initial eval.
