Add placeholder text to Assessment box to guide therapists to better documentation using keywords like: "Educated, Progressed, Modified, Instructed"

Through our Best Practice Committee, we have identified that therapists don't justify the skilled services they provide enough in their documentation. Placeholder text (that would start shaded and then disappear as you begin to type) would be a great way to nudge therapists in the right direction to entering valuable documentation that would demonstrate the skills of a therapist in their daily documentation as well as their Progress Notes/Recertifications
